Tianna Swede, ACSW, Patient Care Services Manager  

Tianna initially worked for Hospice of Santa Barbara as a palliative care social worker from 2005-2009. She returned to HSB in 2011 as the Patient Care Services care manager. Before that, Tianna worked for Kaiser Permanente as a palliative care social worker in Atlanta. She also served as a palliative care social worker at Santa Barbara Cottage Hospital, and as a hospice medical social worker at Visiting Nurse and Hospice Care of Santa Barbara. Tianna has a bachelor’s degree in social work from San Diego State University and a master’s degree in social work from Fordham University. Tianna loves the interdisciplinary team approach to Patient Care Services, because it allows the whole person to be served. In her off time, Tianna enjoys gardening, cooking, walking her dog on the beach, kickboxing, and spending quality time with friends and family.
